Mental Health Passports, And Safety Plans

Mental Health Passports and Safety Plans are digital spaces that we have developed in collaboration with Sheffield Health Partnership University (SHPU) that support your mental health and can help you access the right support when you need it.

What Are They?

The Mental Health Passport is a space for providing an overview of your mental health history and journey in your own words.

You can share a link to your Mental Health Passport with people who support you with your mental health to provide context or information about how you like to be supported. This could be a loved one, health or social care professional. You can remove your consent for any individuals to view the passport at any time.

The Mental Health Passport can be completed as a stand-alone document, but it also works alongside the Safety Plan and the wider My Toolkit.

The Safety Plan is a space for personal reasons for wanting to live. There are no right or wrong answers.

Staying safe when dealing with thoughts of suicide requires a lot of strength. Our Safety Plan aims to capture anything to help staying safe, such as coping mechanisms, support networks (both personal and professional), any risks which may need removing, distractions and more. The safety plan can be a personal tool or shared with trusted friends, family, or professionals to ensure you have support when needed.

The Safety Plan can be completed as a stand-alone document but also works alongside Mental Health Passports and the wider My Toolkit.
